All New Patients
- Patient Information — contact details, emergency contact and how you heard about us
- Health History — current symptoms, past injuries, surgeries, medications and conditions
- Consent to Care — explains chiropractic care and your rights as a patient
- Privacy Notice Acknowledgment — confirms you received our notice of privacy practices
- Financial Policy — outlines payment, insurance and cancellation policies
If It Applies to You
- Auto Accident Questionnaire — for injuries from a car accident
- Work Injury Form — for injuries that happened on the job
- Minor Consent Form — signed by a parent or guardian for patients under 18
- Pregnancy Questionnaire — for expecting mothers
- Neuropathy Intake — for numbness, tingling, burning or balance concerns
- Records Release — lets us request records or imaging from another provider
What to Bring to Your First Appointment

- Photo ID
- Insurance card (front and back), if using insurance
- Claim number and adjuster contact, if your visit involves an auto or work injury
- A list of current medications and supplements
- Recent X-rays, MRIs or reports, if you have them
- Comfortable clothing

Your health information is protected. Forms submitted online are transmitted securely and handled in line with HIPAA privacy requirements. Please do not email completed forms containing personal health details to a general inbox; use the secure upload, bring printed copies, or fax them to our office instead.
